F. Heath Smith IV, LPC-S, LCDC is trained and experienced in counseling individuals, couples, and families. He provides counseling to adolescents and adults presenting with a wide range of issues including relationship difficulties and life transitions. Many employers and government agencies are now requiring substance use/abuse evaluations for individuals that have violated state/federal law, licensure rule, or drug-free workplace employee contract. These violation evaluations often require a counselor with specific training and education in the field of addiction. F. How do I schedule an appointment?
Setting an appointment with Heath Smith, LPC-Supervisor, NCC, LCDC, ADC III, MAC or Alex Lopez, MS, CRC, LPC-Associate is relatively easy. You may contact the office at 956.994.1428 Monday through Friday. In addition to face-to-face counseling sessions, Heath Smith and Alex Lopez provide teletherapy for appropriate counseling issues.
To avoid delays in securing an appointment, please complete our intake packet in its entirety and submit all of the required pages along with proof of identification. After all of the required information is received at our office, then we will contact you to move forward with scheduling an appointment.
For INDIVIDUAL counseling services provided by Heath Smith OR Alex Lopez, complete and submit the intake packet along with proof of identification.
For COUPLES counseling services provided by Heath Smith, both you and your partner will each need to complete and submit the six (6) page intake packet along with proof of identification.

Do you accept insurance?
All counseling services are private as well as private/self pay. This office does not work directly with any insurance carriers. For patients consulting with Heath Smith, if you would like to submit an itemized receipt to your insurance carrier for out-of-network reimbursement, please let the office know and Mr. Smith may provide a mental health diagnosis, if applicable, for insurance purposes. Any diagnosis given creates a public record of a mental health condition. Your signature gives our office permission to communicate with your insurance carrier which, in turn, means your counseling is not confidential. Your insurance carrier will determine how many sessions it will cover for the diagnosis. Additionally, there are some mental health services that many insurance carriers do not cover. It is your responsibility to check with your insurance carrier, prior to your appointment, to determine if services provided by Heath Smith will be covered.
