     ​F. Heath Smith IV, LPC, LCDC is trained and experienced in counseling individuals, couples, and families. He provides counseling to adolescents and adults presenting with a wide range of issues including depression, anxiety, addictions, ADHD, relationship difficulties and life transitions. Heath is passionate about holistic wellness that incorporates the individual’s personal growth mentally, emotionally, physically, spiritually, and volitionally. He has a strong interest in assisting people to create lives of meaning and purpose through the hard work of therapeutic exploration.

     Conflict, infidelity, stress, and other issues take a toll on marriages. Family Counseling
     When one person in the family is struggling, their struggles affect the entire family. Marital conflict, parent/teen battles, and life trauma(s) all are common to people who seek family counseling. Family counseling has been shown to be highly effective in dealing with issues that affect the adolescent as well as the family as a whole.

Addiction Counseling  
     Drugs, alcohol, gambling, or other compulsions can take control until a person or couple feels like there is no escape. There are many paths to recovery and with addiction every person’s problem is unique. Some individuals will require residential rehabilitation but many find that outpatient addiction counseling helps them find the recovery they need for themselves and their family.

Substance Use/Abuse Evaluations and Services
     Many employers and government agencies are now requiring substance use/abuse evaluations for individuals that have violated state/federal law, licensure rule, or drug-free workplace employee contract. These violation evaluations often require a counselor with specific training and education in the field of addiction. F. Heath Smith IV is a licensed and board certified prevention and addiction specialist as well as a Department of Transportation qualified Substance Abuse Professional and provides substance use/abuse evaluations for:

     • Dental professionals,
     • Pharmaceutical professionals,
     • Department of Transportation covered employees,
     • Drug-free workplace industry violations,
     • DWI/DUI.

Frequently Asked Questions

How do I schedule an appointment?
Setting an appointment with Heath Smith is relatively easy. You may contact the office at 956.994.1428 Monday through Friday for information.  In addition to face-to-face counseling sessions, Mr. Smith provides teletherapy for appropriate counseling issues.  Client intake forms will need to be completed and submitted to our office prior to your initial session.  Please see link below for our office six (6) page intake packet.

Email the completed six (6) page intake packet along with proof of identification to cofhs4@gmail.com.  
For INDIVIDUAL counseling services, complete the six (6) page intake packet and submit it along with proof of identification.
For COUPLES counseling services, both you and your partner will each need to complete the six (6) page intake packet and submit it along with proof of identification.  

For all incomplete and/or illegible (dark/blurry) documents submitted to our office, please know you will be contacted via telephone and/or email, asked to complete the packet in its entirety, and resubmit legible documents to our office.  To avoid delays in securing an appointment, we encourage you to complete the intake packet in its entirety and submit all required documents which includes proof of identification.

Do you accept insurance?
All counseling services are private as well as private pay.  This office does not work directly with any insurance carriers.  If you would like to submit an itemized receipt to your insurance carrier for out-of-network reimbursement, please let the office know and I will provide a mental health diagnosis, if applicable, for insurance purposes.  Any diagnosis given creates a public record of a mental health condition.  Your signature gives me permission to communicate with your insurance carrier which, in turn, means your counseling is not confidential.  Your insurance carrier will determine how many sessions it will cover for the diagnosis.  Additionally, there are some mental health services that many insurance carriers do not cover.  It is your responsibility to check with your insurance carrier, prior to your appointment, to determine if services provided by me will be covered.
Is therapy confidential?
All counseling information is confidential and protected by federal and state law with some limitations. Your counseling is confidential except in some situations such as suicide, homicide, abuse, neglect or exploitation of a child, the elderly or person with a disability or state/federal court order.
Why The Sendero Group, PLLC?
F. Heath Smith IV is often asked why he named his company The Sendero Group, PLLC. Sendero is a Spanish term which means path/pathway. The Sendero Group, PLLC was born out of a desire to walk alongside individuals and their families in this path we call life and help them discover the power of healing, health, recovery and strength.
